After the Team Captain has registered the team, all team members will be sent a waiver to fill out via Google Form

Registration is to be completed by one team member for the entire team, listed as the “Team Captain” below. If you do not have a team but still wish to attend, please use the form below the team sign up form.

Shirts must be purchased by April in order to have them delivered to your team by the event. The final deadline for shirts is April 16.

Waivers are due via Google Form the day before of the Ultimate Frisbee Fest, Friday, April 16, 2021.
